Registered Nurse (RN) - Acute Rehab - NJ Location

Job Title: Registered Nurse (RN) - Acute Rehab 

Location: New Jersey (Traveler RN Role - Multiple Locations) 

Schedule: 

• Hours Per Week: 36 

• Hours Per Day: 12 

• Days Per Week: 3 

• Shift: 7:00 AM - 7:30 PM, 3 x 12-hour day shifts 

• Every other weekend required, holidays as needed 

• No on-call requirement, potential float to med-surg telemetry units if necessary 

Job Summary: 

The Registered Nurse (RN) - Acute Rehab delivers exceptional care in a 40-bed acute rehab facility, supporting patients in recovery and rehabilitation. The role involves assessing, planning, implementing, and evaluating individualized care plans to meet the specific needs of patients. 

Key Responsibilities: 

• Assess and evaluate patient conditions to assist physicians in developing care strategies. 

• Plan, implement, and document nursing care tailored to patients’ rehabilitation needs. 

• Identify patient needs and prioritize care through assessment, intervention, and evaluation. 

• Develop and coordinate individualized care plans based on initial and ongoing assessments. 

• Perform therapeutic nursing interventions and manage patient care effectively. 

• Document care in alignment with standards of practice, acuity, and clinical priorities. 

• Advocate for evidence-based practices and foster innovations in care delivery. 

• Deliver patient-centric care in accordance with professional nursing practice standards. 

• Collaborate with interdisciplinary teams to optimize rehabilitation outcomes. 

• Other duties as assigned. 

Required Skills & Experience: 

• Minimum of 2+ recent years of strong acute rehab experience (hospital-based required). 

Preferred Skills & Experience: 

• Minimum of 2 years acute rehab and med-surg experience highly preferred. 

Education: 

• Required: Associate of Science in Nursing (ASN). 

• Preferred: Bachelor of Science in Nursing (BSN). 

Certifications & Licensure: 

• Required: 

• BLS (Basic Life Support) - AHA. 

• NIH Stroke Scale (NIHSS). 

• NJ State Professional Registered Nurse license or multi-state compact license. 

• Preferred: None specified. 

Languages: 

• English (Speak, Read, Write).
