Job Title: LPN - Long Term Care
Duration: 13 weeks+ contract (Extendable)
Work Location: Iron River, MI
Shift: Monday-Friday, 8 hrs/day

Position Summary:

  • Observe each resident continuously, staying alert to changes in mental and physical well-being. Notify the Supervisor of any pertinent observations, discuss the need to notify the physician, and receive the Resident Status Report from the off-going shift. Provide the on-coming shift with a detailed report of each resident’s status and events of the shift.
  • Record narcotics on a 30-day basis.
  • Accurately receive, transcribe, and carry out physician’s orders.
  • Administer medications and treatments as ordered by the physician, documenting all actions via a computerized system.
  • Perform wound care and demonstrate knowledge of wound vacs.
  • Perform regular unit duties such as checking appointments for Physical, Occupational, and Speech Therapy, reviewing the activity board, reminding staff of ambulation lists, and documenting weights, blood pressures, and nutritional charts.
  • Monitor LNA documentation, update the LNA clipboard, and ensure infection control practices on the unit. Make sure the unit is clean and hazard-free. Follow the chain of command to report any problems to the Nurse Manager/Nursing Supervisor, encouraging LNA observations and including them in documentation and reports.
  • Admit, discharge, and transfer residents while completing all necessary documentation and notifications.
  • Complete a 24-hour report for assigned unit(s), review new orders, track residents on antibiotics, precautions, etc., and maintain consistent communication with the Nurse Manager/Nursing Supervisor about resident issues or staff concerns. Differentiate between acute and non-emergency problems. Attend mandatory staff/educational meetings and non-mandatory meetings as assigned.
  • Communicate with residents’ families to resolve concerns, notify them of new orders, and obtain consent when necessary.
  • Receive incoming calls and determine the appropriate response.
  • Follow State and Federal Regulations regarding the storage, usage, and documentation of controlled medications, including counting at shift changes. Notify the appropriate Supervisor if discrepancies are found.

Submission Requirements:

  • 6 months of experience in LTC – Required
  • BLS – Required

Education:

  • Valid LPN license to practice in Michigan
  • Primary Source Verification required

Skills:

  • At least 2 years of SNF/LTC or Med Surg experience
  • BLS (AHA) certification
  • COVID Vaccine required (Medical & Religious Exemptions considered on a case-by-case basis)
  • Ability to work in a high-paced, stress-prone environment

State License Requirements:
