Job Title: Licensed Practical Nurse (LPN) – Long-Term Care

Location: Falmouth, MA
Duration: 13 Weeks
Shift: Evening Shift | 5x8 Hours | 3:00 PM – 11:30 PM

Compensation

Traveler Pay: $52–$54/hr (Based on Experience)
Local Pay: $43–$45/hr (Based on Experience)

Position Overview

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care team. The LPN will provide direct resident care, oversee nursing assistants, administer treatments, maintain accurate documentation, and collaborate with physicians and interdisciplinary healthcare professionals to ensure quality resident outcomes.

Key Responsibilities

Resident Care & Clinical Support

  • Provide nursing care in accordance with individualized care plans and facility policies.
  • Monitor resident conditions and report significant changes to the healthcare team.
  • Administer medications and treatments as prescribed.
  • Assist with resident admissions, transfers, and discharges.

Documentation & Record Management

  • Complete and maintain accurate resident records, charts, and required documentation.
  • Record physician orders and ensure timely transcription to resident charts, medication records, treatment plans, and care documentation.
  • Document nursing notes in a clear, accurate, and descriptive manner reflecting care provided and resident responses.

Team Leadership & Coordination

  • Direct and supervise Nursing Assistants in the delivery of resident care.
  • Coordinate daily activities, assignments, and resident services with nursing staff and support personnel.
  • Participate in shift planning to ensure efficient and effective resident care delivery.
  • Promote teamwork and maintain effective communication among interdisciplinary team members.

Physician & Family Communication

  • Communicate effectively with physicians regarding resident care needs and treatment plans.
  • Receive and accurately document physician telephone orders according to facility policy.
  • Encourage timely physician review and documentation of treatment plans, progress notes, and orders.

Compliance & Quality Assurance

  • Adhere to all federal, state, and facility regulations governing long-term care services.
  • Support quality improvement initiatives and maintain compliance with nursing standards and best practices.
  • Promote resident safety, dignity, comfort, and quality of life at all times.

Required Qualifications

Education

  • Graduate of an approved Licensed Practical Nursing (LPN) Program or accredited nursing school.

Licensure & Certifications

  • Active Massachusetts Licensed Practical Nurse (LPN) License.
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA).

Skills & Experience

  • Previous Long-Term Care, Skilled Nursing, Rehabilitation, or Geriatric Nursing experience preferred.
  • Strong clinical assessment and medication administration skills.
  • Excellent communication, organizational, and documentation abilities.
  • Ability to work effectively in a fast-paced team environment.
  • Strong leadership and supervisory skills.
