Position Summary
The Licensed Master Social Worker (LMSW) will provide professional social work services within an inpatient hospital setting, supporting patients in medicine, surgery, or rehabilitation units.
This role focuses on psychosocial assessments, care coordination, discharge planning, and collaboration with interdisciplinary clinical teams to ensure safe and effective transitions of care. The LMSW will work closely with patients, families, and healthcare professionals to address psychosocial needs and connect individuals with appropriate resources.
Key Responsibilities
Patient Assessment & Support
Conduct comprehensive psychosocial assessments for inpatient populations.
Provide emotional support, counseling, and crisis intervention when necessary.
Assess patient needs and develop appropriate care and support plans.
Care Coordination & Discharge Planning
Participate in discharge planning and coordination of care for patients transitioning from hospital to home or other care settings.
Collaborate with physicians, nurses, case managers, and interdisciplinary teams to support patient care plans.
Facilitate access to community resources, social services, and support programs for patients and families.
Documentation & Compliance
Maintain accurate documentation of assessments, interventions, and care plans in the EMR system.
Ensure all services comply with hospital policies, New York State regulations, and professional ethical standards.
Advocacy & Team Collaboration
Advocate for patient needs and support continuity of care across healthcare settings.
Participate in team meetings, case conferences, and quality improvement initiatives.
Support patients and families by providing education and resource guidance.
Required Skills & Experience
Minimum 1 year of experience as a Licensed Master Social Worker.
Experience working with inpatient populations in medicine, surgery, or rehabilitation settings preferred.
Hospital-based social work experience strongly preferred.
Strong communication, crisis intervention, and care coordination skills.
