Job Description:
As a Licensed Practical Nurse (LPN) you will be responsible for providing all geriatric nursing care, treatment, and services in a professional and prescribed manner. You will provide a variety of nursing care for infirm geriatric residents while overseeing and training subordinate staff in specific resident care. You will observe and report resident behavior and physical condition to include administering medical treatment and providing intervention according to the plan of care. In addition; you will act as a lead with limited supervision on assignments. This position works under the supervision of an RN or physician however in the event the LPN position is the charge nurse, the incumbent oversees the functions of all other LPNs on the unit, as well as the Certified Nursing Assistants.
Administers medication and performs treatments on an assigned unit as ordered by the physician and within the scope of the license.
Medications may be in a variety of forms including oral, enteral, parenteral, suppository, etc.
Treatments may include sterile and aseptic wound cleaning and dressing, postural drainage and percussion, tracheostomy, gastrostomy jejunotomy care, and catheter care.
Receives and delivers a variety of information regarding the residents' medical, social, functional, and other needs.
Communicate with the resident, family, and staff as necessary to ensure the resident receives appropriate care from the interdisciplinary team.
Completes the initial admission nursing assessment tool on a newly admitted resident.
Assists with the development of the Minimum Data Set (MDS) assessments, ongoing quarterly reviews, and care plans.
Maintains appropriate clinical and administrative documentation regarding diagnosis, observations, treatment, and summary of resident progress.
Supervises subordinate staff and assists as needed with direct resident care for the completion of Activities of Daily Living (ADLs) including bathing, dressing, eating, toileting, personal hygiene and mobility.
Learns, implements, and monitors Arizona Management System (AMS) methods. Utilizes AMS concepts and tools for problem-solving and work process improvements.
Actively participates in AMS processes such as Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work.
Fully participates in the QAPI program, our Quality Assurance and Performance Improvement, which incorporates AMS concepts.
Conducts and participates in in-service training and staff development programs for regularly employed staff.
May collect and analyze data for quality improvement and infection control programs.
Trains/orients new employees.
Skills Required:
Gerontological/Geriatric nursing theory and practice
1+ years in a Skilled Nursing / Long-term Care facility
Federal, State, and agency laws, rules, and regulations governing general practical nursing practices and the security and control of medical supplies and medications and dispensing
Practical nursing methods and techniques
Observing and reporting the behavior and progress of residents/patients
Applying and interpreting State and agency laws and regulations governing geriatric nursing practices
Assisting in the development and implementation of patient care plans within a team nursing concept
Oral and written communication skills
Listen, observe, assess, and respond to medical emergencies
Evaluate, analyze, make independent decisions, and follow up regarding client health problems
Respond to medical issues/inquiries in a professional manner
Work with professional and non-professional staff to ensure the health and safety of residents
Establish and maintain effective work relationships
License Required:
 Must possess an active license to practice as a Licensed Practical Nurse in the State of Arizona
