Job Summary

"Navitas Healthcare, LLC" is seeking a Virtual Care Coordinator to support telehealth and remote patient monitoring services. This role focuses on continuous virtual observation, patient safety monitoring, and coordination with clinical teams to ensure effective delivery of virtual care services.


Key Responsibilities

  • Monitor patients remotely for safety, behavior changes, and clinical warning signs
  • Provide continuous virtual observation to prevent falls and ensure patient safety
  • Report all clinical changes immediately to RN or physician
  • Document patient behavior, interventions, and escalation events accurately (paper documentation)
  • Support telehealth consultations, scheduled visits, and remote monitoring workflows
  • Ensure telehealth sessions start and run smoothly using approved systems
  • Monitor Epic telehealth consults, follow-ups, and scheduling workflows
  • Assist in triaging and assigning virtual monitoring cases
  • Provide real-time support (“white glove service”) for telehealth technology issues
  • Collaborate with physicians, nurses, IT teams, and vendors for issue resolution
  • Support quality assurance, data tracking, and program performance monitoring
  • Escalate patient safety concerns and equipment issues promptly
  • Maintain compliance with HIPAA and hospital privacy policies
  • Participate in staff meetings, training, and workflow improvement activities

Required Skills & Qualifications

  • Minimum 3 years of direct patient care experience required
  • Minimum 2 years of telehealth or remote monitoring experience required
  • Experience with electronic medical records (Epic preferred)
  • Strong understanding of patient safety monitoring and clinical escalation protocols
  • Experience with quality improvement, data tracking, or telehealth workflow development
  • Strong communication, multitasking, and problem-solving skills
  • Ability to work independently in a fast-paced virtual care environment
  • Associate or Bachelor’s degree in a health-related field preferred
